What should I do if I find my name on the list?
Contact our office to initiate your claims process and determine what documents will be required for verification. Have the following information ready:
• Name as it appears in the list.
• Current address, plus all previous addresses
• Social Security number.
• Insurance policy information, policy numbers, etc.
